To Email PDf Form:
If you use email software such as Microsoft Outlook or Outlook Express, simply click the send
button at the bottom of the form to email it to Crossroads Dentistry.
If you use internet email such as Hotmail, Gmail or Yahoo you will first need to save the PDF form
to your computer and then, once completed, attach the saved PDF to an email message and send to crossroadsdentistry@comcast.net.
